▲The above link is for the Satisfaction Survey of the Instrumentation Center for the first semester of the 114th academic year.
▲All the information in this questionnaire is analyzed in a holistic manner. It will be absolute confidentiality and not be handling individually. Please feel free to answer. 
▲To ensure that each instrument and its technician can more accurately receive feedback from users, the questionnaire can only be filled out for one instrument at a time. If multiple instruments were used, please complete the questionnaire repeatedly for each one.  

▲The list of winners will be announced on 12/30 after the survey is completed. 
The grand prize  → iwalk Power Bank (black/lightning)+711 NT$200 voucher.  (one person)
The second prize  →  Logitech Bluetooth Wireless Mouse (two person)
The third prize  → Uni-President NT$100 Gift Voucher  (three person)
The consolation prize  →   7-11 coffee  voucher. (several people limited quantity)

Winners are kindly requested to collect their prizes at the CFC between 9:00 AM and 5:00 PM from 12/30 to 2025/1/9. Any prizes not claimed by 5:00 PM on 2028/1/9 will be considered forfeited.  
The number of consolation prizes will vary based on the number of participants. The final interpretation of the lottery results and prize items rests with the CFC.
There will be no duplicate winners for the grand prize, second prize, or third prize.
Since participants can fill out the questionnaire multiple times, each person is limited to a maximum of 3 prizes.
